Application Form

Thank you for your interest in our memorial bench program. Your purchase includes the installation of a bench with a commemorative message on an engraved bronzed plaque. Please expect a 16 week lead time on ordering.

Bench installation happens between the months of May and December each year and are dependent on operations and weather conditions. Due to the personalized nature of this program, refunds are not possible.

Please note: The Town of Kingsville reserves the right to make the final decision regarding the location. The location may be changed in the event it becomes unavailable for any reason. A notification will be sent to the applicant if the location is not available and the Town proposes a new location.
