This application will be used to remove a deceased person's name voters' list.

Carefully read the below informaton before proceeding to complete the form.

This form is to be used by an Elector wishing to remove a deceased Elector's name from the Voters' List.  If an Elector requires assistance in completing this Form, they may attend the visit the Town of Kingsville's Municipal Office for assistance. You must carefully complete the Form to ensure the information you provide is true and correct - carefully check your spelling and dates. 

Government Issued Identification

Have valid, government-issued identification readily available for upload as you will need to provide proof of identity and/or citizenship at the end of this Application. If you do not upload sufficient identification, there could be delays, your Application may be rejected, or you may have to attend the Kingsville Municipal Office in person.

A valid Driver's Licence showing your qualifying address is preferred. If you do not have a Driver's Licence, you may choose to upload one of the following ID types:

Proof of Death

You will need to submit a copy of documentation showing Proof of Death. Examples of documents that can be used to prove death include:

  • Burial or Death Certificate 
  • Document or receipt issued by a cemetery or crematorium in Ontario
  • Life or group insurance claim along with a statement signed by a medical doctor
  • Medical Certificate of Death
  • Memorandum of Notification of Death, issued by the Chief of the Defence Staff
  • Notarial Copy of Letters Probate
  • Official Notification from the Office of the Public Guardian and Trustee
  • Program or Notice from a Funeral Ceremony
  • Registration of Death 
  • Statement from a Doctor, Coroner, or Funeral Director
  • Statement of Verification of Death from Veterans Affairs Canada
