Application to Discharge Display Fireworks

In accordance with Fireworks By-law 102-2003, any person wishing to discharge display fireworks must apply for and obtain a permit. Applications must be submitted at least 30 days in advance of the event.

Event Information

Site Plan

You must upload a site plan which shows the:

• separation distances to the public and vulnerable features;
• position of ramps and mortars;
• fallout zone;
• directon of firing;
• significant ground features inlcuding roads, public right of way, buildings and strucutures, overhead obstructions, parking areas, and spectator viewing areas; and,
• an arrow indicating north.

Certificate of Insurance

You are required to provide a Certificate of Insurance which notes the following:

• General Liability Insurance in an amount no less than $5 million dollars;
• The Town of Kingsville as Additional Insured; and,
• A provision for Cross Liability.

Declaration

As the Applicant, I certify the truth of all the statements and representations in this and all other documents submitted. I agree that if a permit is issued not to depart from the plans, routes, specifications or spreading located proposed in this applications. I acknowledge that it is an offence to provide false information or to contravene any conditions contained in the permit, if issued.

Further, I declare that the discharge of the fireworks subject to this Application will conform to the NRCan's "Display Fireworks Manual" in accordance with O.Reg. 213/07: Fire Code.
