If you are filling out the Adult Waiver, Name 1 is the adult's name. If you are filling out the Child Waiver, Name 1 is the child's name. If the photo has multiple children from the same family and only signature is required, put all their names on the Name 1 text field. If the photo has multiple children from different families and requires multiple signatures, add additional names.
