Spay or Neuter Voucher Application (Feral Cats)

Important Notes:

• Vouchers are issued in the amount of $75.00 and are valid for 90 days from Date of Issue;
• Vouchers may only be used for spays/neuters;
• Vouchers may not be combined; and,
• All Feral Cats that undergo a spay/neuter procedure will automatically be eartipped to mark the cat as feral.

HOLDER OF VOUCHER

The holder of this Voucher acknowledges that this Voucher is issued by The Corporation of the Town of Kingsville without warranty and that surgery bears inherent risk to the animal. The individual does hereby waive any and all claims that he/she has or may in the future have against The Corporation of the Town of Kingsville, its officers, servants and agents (the “Releasees”), and to release the Releasees from any and all liability for any loss, damage, expense or injury including death that they may suffer resulting either directly or indirectly from their use of or failure to use this Voucher due to any cause whatsoever, including negligence, breach of contract, or breach of any statutory or other duty of care on the part of the Releasees. The holder of this Voucher further agrees to hold harmless and indemnify the Releasees from any claims or demands resulting from any damage or injury whatsoever, whether to them or third parties, which might be made against the Releasees resulting from the use of or failure to use this Voucher.
